| The HKC maintains a confidential medical record for every patient. Your medical record contains complete information about all treatment and services you have ever received at HKC All medical records kept at the HKC and their content are considered confidential information and can not be released to any person except under explicit medical or legal order as dictated by law. However, upon discharge, all patients will receive a summary sheet related to diagnosis and treatment (and a copy of their paraclinical data upon request). |

When needed for your continued care, we will hand in copies of other documents such as operation reports, consultations, prognosis and observations to health care providers upon request from that provider at no cost.To obtain a complete copy of your medical record, please submit the providers request form to the Medical Records unit |
